Any mental or physical status examinations that you’ve undergone after you became disabled need to be represented in your medical evidence. If you have records from these kinds of evaluations before your disability, it is also important to include them, as they demonstrate how the disability affects you.

#3. Diagnosis

This one seems pretty obvious, but it is an incredibly important thing to include in your medical evidence. Without the diagnosis, it will be very difficult to prove your disability.
